Qualifications:
A graduate of a college or university with a bachelors degree in a human service field and has two years full-time, post-bachelors degree accumulated MH/DD/SA experience with the population served, or college graduate with a degree other than human service with four years of full time post graduate experience and/or a substance abuse professional who has two years of full-time, post bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling.
Responsibilities:
Document appropriate clinical information in medical record in a timely manner including but not limited to:
1. Monitoring the status of the recipients’ progress and the effectiveness of the strategies and interventions outlined in the PCP.
2. Submission of service notes within 24hrs of service delivery.
3. Offer coping skills to consumers served
4. Therapeutic interventions for the consumers in group setting
5. Crisis management
6. Completion and submission of all documentation required under the day treatment service definition
7. Ensure daily documentation is completed for shift (point sheets & behavioral tracking logs)
8. Provide transportation to consumers within the day treatment program as needed
9. Facilitation of therapeutic groups
10. Close out the shift by ensuring no materials are left out at the end of the shift, ensure the group areas are clean
11. Ensure incidents within the day treatment program are reported to supervisor immediately
12. Participate in administrative/clinical supervisions with supervisors at least once monthly
13. Follow through with recommendations from immediate supervisor
