Position Overview
We're looking for a motivated, compassionate Physical Therapist (PT) to join our growing home health team in DeKalb County. Available in Part-Time, Full-Time, and Contract formats, this role gives you the flexibility to build a schedule that fits your life while making a real difference — helping patients regain independence and improve their quality of life in the comfort of their own homes.

Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Deliver skilled PT services in patients' homes in accordance with physician orders
- Modify treatment plans based on patient progress and outcomes
- Supervise and collaborate with Physical Therapist Assistants (PTAs) as needed
- Educate patients and caregivers on exercises, home safety, and injury prevention
- Accurately and timely document evaluations, visits, and patient progress
- Communicate effectively with physicians and the interdisciplinary care team
Qualifications
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Current PT license in the state of Georgia (or eligible)
- Strong communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation for travel between patient homes
- Home Health experience preferred, but new graduates and clinicians new to home health are strongly encouraged to apply
