This position reports directly to the President & CEO and responsible for the coordination, implementation, and administration of an activites related to Veterans Directed Care Program and other programs and services designated to support Veterans. This includes providing :
Case Management
Care Coordinating
Consulting Services to Veterans and their family members. Candidate must be able to advocate for the client. Must have an understanding of the Veterans Administration and Healthcare.
DUTIES:
Provide detailed VDC program Orientation to Veterans
Self Direction
Plan Creation
Service Provision
Budget Management
Conducts Virtual & Home Based visits to perform assessments
Develop Service Plans
Identifies functioning level/& Sources of support
Development of care plans, coordinating care, making sure the candidate is enrolled in VDCP and other DAAA administered programs.
Assist processing and reconsiliation of monthly VDC program invoices
Development of internal processes and systems that identify, evaluate, and transition Veterans into the VDC program. Maintains an active list of Veterans in the data Base
Qualifications:
Bachelors Degree from a accredited 4year college
Masters Degree and community health worker certification preferred, Military experience preferred, Must have 3-5 years experience working with the Veteran population
