The Admissions Coordinator for Centralized Admissions will oversee all referral operations related to the intake and admission for prospective residents across our facilities. This individual will work with a team, build relationships with external company liaisons, ensure compliance with regulations, and maintain exceptional service standards that align with our commitment to excellence.
Key Responsibilities:
- Oversee the referral process, from initial inquiry to resident placement, ensuring a smooth and efficient workflow.
- Ensure timely responses to inquiries from hospitals, healthcare providers, and families.
- Work collaboratively with facility staff and liaisons to coordinate and confirm placements.
- Build and maintain strong relationships with external team, including hospitals, social workers, and case managers.
- Represent the company professionally to external stakeholders to enhance reputation and trust.
- Ensure all admissions processes comply with state and federal regulations, as well as company policies.
- Monitor and audit referral records for accuracy and completeness.
- Address and resolve issues related to denied or pending admissions.
- Eligibility of payer sources whether it be Medicare, Medicaid, Manage Care or Private Pay.
- Collaborate with leadership to develop and implement strategies to meet occupancy and revenue goals.
- Prepare and present reports on admissions metrics to the executive team.
Qualifications:
Education & Experience:
- Minimum of 5 years of experience in admissions, preferably in a healthcare or skilled nursing setting.
- Proven experience in team management and leadership.
- Strong organizational and multitasking abilities in a fast-paced environment.
- Exceptional communication and interpersonal skills to effectively interact with families, referral sources, and internal teams.
- Proficiency in CRM software, electronic health records (EHR), and other admissions-related systems.
- Analytical skills to evaluate data and drive decisions.
- In-depth knowledge of healthcare regulations and skilled nursing admission protocols.
