Position Duties and Responsibilities:
The LPN for our Home care team responsibilities will include, but are not limited to the following:

Under the direction of the registered nurse, provides end-of-life care to patients and families.

Monitor and evaluate medication needs and administration of patients, including symptom management, supplies, and equipment needs.

Documents patient visits.
-
Communicates progress and needs of patients and families to other interdisciplinary team (IDT) members promptly and professionally.

Assess patient safety and provide education on safety issues to patients and families.
-
Refer patients to other disciplines as needed.
-
Educate patients and families on using durable medical equipment (DME) appropriately.
-
Assist CNA in providing CNA services as needed.
-
Provide RN Case Manager and triage with daily reports of patient progress.
-
Deliver supplies for RN Case Manager as needed.
-
Assists RN Case Manager in preparation of IDT review during team meetings and via reports.
-
Provide HHA services in the absence of HHA while on-call during visits.
-
Satisfactorily complete competency requirements for this position.

Performs other job duties as requested.
Work Schedule:?

Monday through Friday 8AM-5PM
Travel Requirement: You must have a reliable vehicle and be willing to be dispatched throughout our service areas.

Requirements:
Education & Experience:

Minimum of one [1] year experience in a medical/surgical inpatient unit
-
Hospice/Home Health experience preferred

Ability to physically access any home in the service area, i.e., navigate stairs, narrow spaces, lack of air conditioning and heat, etc.

Ability to remain calm and troubleshoot/handle patient/family crisis situations
