Responsibilities
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote comfort and independence
- Monitor vital signs including blood pressure, pulse, temperature, and respiration; document findings accurately using electronic health record (EHR) systems
- Support medical procedures by preparing patients and assisting with tasks like catheter care, medication administration under supervision, and basic wound care
- Maintain patient safety through proper manual handling techniques, safe lifting practices, and use of equipment like Hoyer lifts and transfer devices
- Observe and report changes in patient condition promptly to nursing staff; document observations clearly in medical records
- Uphold infection control standards by practicing aseptic techniques and adhering to HIPAA regulations to protect patient confidentiality
- Collaborate effectively with interdisciplinary teams across settings such as hospitals, long-term care facilities, assisted living communities, hospice care environments, and home health agencies
Qualifications
- Valid certification as a Certified Nursing Assistant (CNA) or equivalent credential recognized by state licensing boards
- Experience working with diverse populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia-related conditions such as Alzheimer’s disease
- Knowledge of medical terminology, anatomy, physiology, and basic life support (CPR) certification required
- Familiarity with electronic health records (EHR/Epic/eClinicalWorks/Cerner) systems for accurate documentation
- Prior experience in long-term care facilities, hospitals (including ICU or trauma centers), hospice or home care is highly valued
- Ability to perform heavy lifting safely and demonstrate manual handling skills in accordance with safety protocols
- Strong communication skills to effectively engage with patients, families, and healthcare team members
