As the Community Outreach Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:

Provide crisis, individual, and family counseling on a short-term basis to residential and non-residential youth who have: run away, are at risk of running away, have been kicked out of their homes, or are homeless.

Share 24-hour on-call responsibilities on a rotating schedule.

Deliver comprehensive case management services, including referrals, advocacy, consultations, aftercare, and follow-up support.

Assist youth and their families by offering guidance, education on legal rights and options, and supportive encouragement to help them achieve established goals.

Conduct screenings and assessments, develop individualized service plans, and coordinate parental notification.
Community Outreach Coordinator Minimum Salary: $17.00 / Hour
Community Outreach Coordinator Maximum Salary: $19.00 / Hour
Community Outreach Coordinator Schedule: Monday/Wednesday/Friday 8:00AM - 3:30PM; Tuesday/Thursday 12:30PM - 8PM; possible weekends and weeknights, subject to call
Qualifications:

High School Diploma or GED
-
1 to 3 Years experience working with at - risk youth.

Valid NYS Driver's License
