Meadowview Rehabilitation & Nursing Center is seeking a PRN Physical Therapist (PT) to join our dedicated in-house rehabilitation team. This is an excellent opportunity for a therapist looking for flexible scheduling while making a meaningful impact in the lives of our residents.

Responsibilities
Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services to improve mobility, strength, balance, and overall function
Collaborate with physicians, nursing staff, therapists, and families to achieve optimal outcomes
Complete accurate and timely documentation
Participate in care planning and interdisciplinary team meetings
Ensure compliance with all regulatory and facility standards
Qualifications
Graduate of an accredited Physical Therapy program
Current Pennsylvania Physical Therapist license in good standing
SNF/LTC experience preferred, but not required
Strong communication and clinical skills
Experience with Net Health and PointClickCare (PCC) preferred
