Position Overview
The Weekend Triage Nurse serves as the primary clinical point of contact during assigned weekend hours. This role answers incoming calls, assesses patient needs, provides phone triage, and coordinates the appropriate response from the on call team.
The ideal candidate is comfortable working independently, has strong assessment skills, communicates well with patients and families, and can effectively prioritize multiple requests throughout the day.
While this is primarily a triage role an occasional patient visits may be required based on patient needs and staffing demands.
What You'll Do
- Answer incoming calls from patients, families, caregivers, facilities, referral sources, and staff
- Assess symptoms and changes in condition using clinical judgment
- Provide symptom management guidance and support by phone
- Follow clinical protocols to determine when an in person visit is needed
- Assign visits to on call nurses and coordinate weekend patient care needs
- Communicate with physicians, facilities, pharmacies, and other care partners as needed
- Document triage interactions and care coordination activities
- Follow up on patient concerns to ensure needs have been addressed
- Assist with administrative and quality improvement projects during periods of lower call volume
- Complete occasional patient visits when needed
Qualifications
- Current Colorado RN license in good standing
- Minimum of two years of hospice nursing experience is required
- Previous triage, on call, home health, emergency department, critical care, or hospice experience preferred
- Excellent assessment and critical thinking skills
- Strong communication and customer service skills
- Ability to work independently and make sound clinical decisions
- Comfortable using electronic medical records and technology platforms
- Valid driver's license and reliable transportation
Pay: $40.00 - $48.00 per hour
