The Guardian Representative/Case Manager (part-time) is responsible for managing a case load of individuals under guardianship with responsibilities including but not limited to: assisting individuals with decisions regarding medical procedure recommendations and residential placement.
Responsibilities:
- Visit ward's at least monthly.
- Document all activity performed on the ward's behalf in the ward management database within 10 days of activity being performed.
- Periodically assess the ward's functional capabilities.
- Assess and monitor care provision to wards in the areas of residential and social.
- Authorize expenditures for wards based upon maintaining health, safety, and well-being.
- Coordinate ward’s medical care and mental and social services.
- Complete court reports in a timely manner.
- Liaison with court when needed regarding status of cases.
- Maintain caseload-related statistics.
Education and Experience Requirements:
- Bachelor’s degree in Social Work or related field.
- Must have at least 2 years of experience in case management or related experience.
- Must successfully undergo a Level 2 criminal background and credit history.
- Must have general understanding of human services and public benefits.
- Must enjoy working with older adults and persons with disabilities.
- Requires regional travel approximately a 100 mile radius from central office
- Strong verbal and written communication skills.
- Strong knowledge of Microsoft Office, including Word, Excel, Adobe Acrobat Pro and Outlook.
- Ability to learn new computer database management systems.
- Ability to work independently with minimal oversight, manage large workloads and keep deadlines.
This position is part-time at 25 hours per week, with the schedule to be determined by the Manager and Employee.
