Job Summary
We are seeking a compassionate and dedicated Hospice RN Case Manager to join our team and provide exceptional end-of-life care to patients and their families. Duties
- Conduct thorough patient assessments to determine eligibility, needs, and goals for hospice care, including vital signs, physiological status, and symptom management.
- Develop individualized care plans that incorporate pain management, symptom control, psychosocial support, and spiritual considerations.
- Coordinate interdisciplinary team activities to ensure comprehensive patient support across inpatient, outpatient, and post-acute settings.
- Manage discharge planning and transition of care from hospital or clinic to home or hospice facility, ensuring smooth continuity of services.
- Monitor patient progress through regular evaluations, adjusting care strategies as needed while maintaining compliance with HIPAA regulations and medical records documentation standards.
- Collaborate with physicians, social workers, therapists, and other healthcare professionals to optimize patient outcomes in complex cases involving critical care, infectious disease management, or behavioral health issues.
- Educate patients and families on disease processes, medication regimens, and advance directives to empower informed decision-making.
Skills
- Proven experience in critical care environments such as ICU, emergency room, or trauma centers with a strong understanding of physiology and acute care protocols.
- Familiarity with Level I Trauma Centers and Level II Trauma Center operations enhances your ability to manage complex cases effectively.
- Extensive background in hospital medicine, medical-surgical units, or post-acute care settings including skilled nursing facilities or outpatient clinics.
- Knowledge of managed care practices, utilization management processes, diagnostic evaluation techniques, and discharge planning procedures.
- Strong patient assessment skills combined with proficiency in medical terminology and medical records management.
- Ability to handle diverse clinical scenarios involving geriatrics, pediatrics (PICU), behavioral health issues, infectious diseases, or emergency medicine situations.
- Excellent communication skills for patient service delivery while maintaining strict adherence to HIPAA privacy standards.
