Overview
We are seeking a dedicated and compassionate OASIS Registered Nurse (RN) to join our Home Health team in Brevard county, Fl. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background and the ability to work independently while maintaining effective communication with patients, families, and healthcare teams.
Responsibilities
- Conduct comprehensive patient assessments to develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and adjust care plans as necessary.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Collaborate with interdisciplinary teams to coordinate care and ensure optimal patient outcomes.
- Document all patient interactions, assessments, and interventions accurately in electronic health records.
- Assist with pain management strategies and provide urgent care when needed.
- Utilize knowledge of ICD-9 coding for accurate documentation of diagnoses.
- Maintain compliance with all regulatory standards and best practices in home health care.
Requirements
- 1 year Medicare OASIS Home Care required.
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in skilled home care.
- Experience working Wellsky is a plus.
- Excellent communication skills and the ability to build rapport with patients and families.
- Strong organizational skills and the ability to manage multiple tasks effectively.
