Case Manager - LVN – Days
Start Dates: ASAP
Orientation Dates: ASAP ( Paid Orientation—3 days)
- 13-week contract
- 40 hours - 5 x 8 ’s
- Shift: Days - 9am-5:30pm with alternating weekends
- Patient ratios: Within Guidelines of CA State Law
- Float Requirements: None
- Is RTO Accepted? What is the Max: Up to 4 days max RTO approved
- On Call Requirement? NO
- Certs: BLS, LA County Fire Card
- License: Active CA LVN License
- Experience:
- Required Work Experience: 1 year of inpatient/hospital Case Manager experience and 2 years of LVN experience
- Experience required: Discharge Planning, Case Coordination, will be working closely with the Case Manager and Nurses.
- More of a Coordinator. Nothing bedside, someone who has experience with placements, dispositions, home comfort.
- Excellent and clear communication skillset required
- Hospital needs clear and meaningful communicators to improve their outcomes
- Travel Experience not required but preferred
Pay: $2,000.00 per week
