Eldorado Rehab & Healthcare is seeking a detail-oriented and motivated MDS Nurse Manager (RN or LPN) to join our leadership team. This is an excellent opportunity for an experienced nurse who is passionate about accurate assessments, strong care planning, regulatory compliance, and improving resident outcomes.
In this role, you will be a key part of the interdisciplinary team, ensuring timely and accurate MDS completion, supporting reimbursement integrity, and driving high-quality, person-centered care in a collaborative and supportive environment.
Registered Nurses (RNs): $15,000 Sign-On Bonus
Licensed Practical Nurses (LPNs): $10,000 Sign-On Bonus
Paid out over one year.

Oversee and complete MDS assessments in compliance with regulatory requirements
Ensure accurate documentation supporting care plans and reimbursement
Collaborate with nursing, therapy, and administrative teams
Monitor and update care plans based on resident condition changes
Maintain compliance with Medicare/Medicaid regulations and facility policies
Participate in interdisciplinary care planning and meetings
Support survey readiness and quality assurance initiatives
Active Illinois RN or LPN license in good standing
Prior MDS coordination and care planning experience required
Strong understanding of Medicare/Medicaid regulations and reimbursement systems
Excellent communication and interpersonal skills
Highly organized with strong attention to detail
Ability to manage multiple priorities in a fast-paced environment
Availability to work Monday–Friday, 8:00 AM–4:30 PM
