Job Summary
Abilities Plus is seeking a compassionate, organized, and dedicated Qualified Intellectual Disabilities Professional (QIDP) to support individuals with intellectual and developmental disabilities in achieving their highest level of independence and quality of life. The QIDP works directly with assigned individuals to assess needs, develop and implement person-centered programs, coordinate services, monitor progress, and ensure compliance with state regulations, funding source requirements, and agency standards. This position serves as an advocate, liaison, and resource for individuals, families, community partners, and agency staff while promoting the mission and values of Abilities Plus.
Essential ResponsibilitiesAgency Leadership & Administration
- Demonstrate and uphold the mission, values, and ethical standards of Abilities Plus.
- Ensure program delivery complies with funding source requirements, licensing standards, and agency policies.
- Maintain QIDP certification requirements by completing required continuing education annually.
- Promote a culture of workplace safety by following safety procedures, reporting hazards, and participating in safety training.
- Perform additional duties as assigned to support agency operations.
- Chair monthly Case Coordination/QIDP meetings.
- Review Implementation Strategies to ensure accuracy and completeness.
- Conduct annual reviews of Therap documentation and Case Coordination policies.
Case Management & Program Development
- Obtain Person-Centered Plans (PCPs) from the Independent Service Coordination (ISC) agency and develop Implementation Strategies for assigned individuals.
- Coordinate, implement, and monitor individualized service plans and support strategies.
- Maintain timely and accurate progress notes (T-Logs) and client documentation.
- Review monthly program progress notes for assigned individuals.
- Complete annual assessments, including ICAPs, and update information in Therap.
- Conduct initial and annual Individual Service Plans (ISPs) for non-funded and private pay participants in Work Services and Community Day Services.
- Train Direct Support Professionals (DSPs) on the use of Therap documentation.
- Serve as a liaison between individuals served, families/guardians, referral sources, and agency programs.
- Connect individuals and families with appropriate community resources and services.
- Participate on agency and community committees as assigned.
- Attend high school transition meetings to provide information about Abilities Plus services.
Direct Support Services
- Provide transportation for CILA medical appointments as needed.
- Support individuals with self-care training and skill development.
- Assist with social, recreational, and community integration activities when appropriate.
Qualifications
Required:
- Bachelor's degree in Psychology, Social Work, or a related Human Services field.
- Minimum of one (1) year of experience working with individuals with intellectual and developmental disabilities.
- Basic computer proficiency.
- Valid driver's license, reliable transportation, and proof of automobile insurance.
- Motor Vehicle Record (MVR) with no more than one moving violation within the past two years.
- Successful Criminal Background Check with no felony convictions.
- Clear record on the Nurse Aide Registry.
Physical Requirements
This position requires the ability to:
- Complete written documentation accurately and timely on a continuous basis.
- Communicate effectively verbally and in writing.
- Work independently while exercising sound judgment.
- Interact continuously with individuals with developmental disabilities, families, and community partners.
- Travel occasionally between service locations and transport individuals as needed.
- Frequently sit while completing documentation and meetings.
- Occasionally lift up to 15 pounds and pull up to 5 pounds.
Work Environment
- Moderate noise level.
- Occasional exposure to blood or other bodily fluids, airborne particles, fumes, or cleaning chemicals.
- Requires close, distance, peripheral, and depth vision, with the ability to adjust focus as needed.

Pay: $25.00 - $27.00 per hour
