The Case Manager provides individualized, trauma-informed support to individuals experiencing homelessness to assist them in obtaining and maintaining safe, stable housing. This position works collaboratively with clients to identify barriers, connect to resources, increase income and benefits, and develop goals that support long-term housing stability and personal well-being.
ESSENTIAL JOB FUNCTIONS
· Provide individualized case management services using evidence-based and trauma- informed care to support clients in developing and achieving goals related to housing stability, income growth, employment, benefits access, and overall wellness.
· Assist clients in navigating complex service systems by providing advocacy, information, referrals, and coordination with community partners, including mental health providers, healthcare providers, and other social service agencies.
· Maintain regular client engagement based on assessed needs, program requirements, and funding guidelines.
· Develop, implement, and monitor individualized service plans in collaboration with clients, outreach staff, shelter staff, and other members of the Client Services team.
· Collaborate with other case management professionals and community providers to ensure coordinated services and continuity of care for shared clients.
· Provide crisis intervention, problem-solving support, and resource coordination as needed while maintaining appropriate professional boundaries.
· Maintain accurate and timely client documentation within the Homeless Management Information System (HMIS) database and ensure compliance with agency policies, Continuum of Care (CoC) requirements, and HUD standards.
· Maintain complete and confidential client records in accordance with organizational policies, funding requirements, and applicable regulations.
· Build trusting, respectful relationships with clients through a compassionate, trauma-informed approach while maintaining appropriate boundaries and professional expectations.
· Participate in team meetings, community meetings, trainings, and other agency activities as assigned by the supervisor or agency leadership.

- Attendance is an essential function of the job.
- Accurately track and report hours worked in a timely fashion using agency provided tools.
- Understand, believe, and advocate for the mission of the NSKS.
- Maintain strict standards of client confidentiality.
- Adhere to agency policies, procedures, and a professional code of ethics.
- Assist in fostering good public relations with the agency.
- Participate in regular staff meetings, staff training programs, supervisory sessions, and accept the responsibility for aiding in the development of positive team relationships.
- Actively participating in ongoing training as needed to increase skills and to meet all certification standards and credentialing policies.
- Schedule flexibility including early mornings and evenings to provide client services when needed.
- Must maintain up-to-date contact information.
EDUCATION AND EXPERIENCE
- Two or more years of experience providing case management, housing navigation, outreach, or related direct services to individuals and families experiencing homelessness or other significant barriers to stability. Bachelor’s degree in social work, human services, psychology, or related field preferred but not required”
- Demonstrated ability to provide trauma-informed, person-centered support while building trusting relationships with guests and maintaining appropriate professional boundaries.
- Knowledge and familiarity with harm reduction principles, substance use disorders, mental health challenges, and community-based resources.
- Experience with diversion-focused problem solving, mediation, and housing stabilization strategies preferred.
- Strong understanding of the barriers faced by individuals experiencing homelessness and the ability to support guests in identifying goals, accessing resources, and achieving greater stability.
- Interest in innovative approaches to supporting at-risk and vulnerable populations strongly encouraged.
- Bilingual (English/Spanish) a plus.

Pay: $45,000.00 - $55,000.00 per year
