Responsibilities
- Provide speech-language pathology services to clients in the home health/hospice setting, including assessment, diagnosis, and treatment planning.
- Deliver direct patient care, implement treatment plans, and monitor progress toward goals.
- Collaborate with an interdisciplinary team to develop and update the patient’s comprehensive plan of care.
- Document evaluations, treatment sessions, and communication with families and caregivers; ensure compliance with relevant regulations and standards.
- Educate patients, families, and caregivers on communication strategies, devices, and supports; facilitate equipment and device use when appropriate.
- Maintain safe practice standards, infection control, and equipment safety; participate in quality improvement activities as needed.
Requirements
- License: Massachusetts Speech-Language Pathologist license (active, required)
- Certification: Certificate of Clinical Competence (CCC) required; clinical fellows accepted
- Education: Master’s or Doctorate degree in Speech-Language Pathology
- Experience: At least 2 years of SLP experience in home care/home health (required); acute care experience preferred
- Credentials: Valid driver’s license and access to own vehicle required
- Certifications: BLS from American Heart Association required
- Skills: Ability to work independently with minimal supervision; strong communication and collaboration skills; familiarity with rehabilitation equipment and language/communication aids
- Other: Must be able to work in a home-based setting; travel-ready and capable of navigating multiple patient homes
