We are seeking a compassionate, dependable, and professional Licensed Practical Nurse (LPN) to join our growing home care team.
The LPN will play a vital role in supporting client care, caregiver oversight, quality assurance, client assessments, training, and clinical documentation. This position offers the opportunity to work directly with clients, families, caregivers, and community partners while helping ensure the highest quality of care.

Position Summary
The Licensed Practical Nurse (LPN) is responsible for assisting with clinical oversight, client care coordination, caregiver support, assessments, supervisory visits, documentation review, and quality assurance activities.
The LPN will work closely with agency leadership, clients, caregivers, referral sources, and healthcare professionals to help ensure client needs are met safely and effectively.
Essential Duties & ResponsibilitiesClient Care & Clinical Support
- Conduct client assessments and reassessments as assigned
- Participate in development and review of client care plans
- Monitor client health status and report concerns appropriately
- Document observations and nursing interventions accurately
- Identify changes in condition and communicate findings promptly
- Assist with client admissions and service initiation
- Support continuity of care efforts
Caregiver Supervision & Support
- Provide guidance and support to caregivers and direct care staff
- Conduct supervisory visits as required
- Monitor caregiver performance and service delivery
- Assist with competency evaluations and skills validation
- Provide coaching and corrective education when necessary
- Promote compliance with agency policies and procedures
Training & Education
- Assist with employee orientation and onboarding
- Provide in-service education and skills training
- Educate staff on infection control, safety, client care, and documentation
- Reinforce compliance with HIPAA, Medicaid, CLTC, and agency requirements
- Assist with competency assessments and annual training activities
Quality Assurance & Compliance
- Participate in quality assurance and performance improvement activities
- Review documentation for accuracy and completeness
- Identify areas for process improvement
- Assist with audit preparation and compliance monitoring
- Report quality concerns and safety risks
- Support regulatory compliance initiatives
Communication & Coordination
- Communicate effectively with clients, families, physicians, caregivers, and referral partners
- Coordinate services with agency leadership
- Assist with problem-solving and service recovery efforts
- Maintain professional relationships with community partners
Additional Responsibilities
The LPN may also:
- Participate in community outreach activities
- Assist with healthcare fairs and community events
- Support marketing and educational initiatives
- Help represent the agency at networking opportunities
- Provide emergency staffing support when necessary and appropriate
QualificationsRequired
- Current South Carolina Licensed Practical Nurse (LPN) License
- Graduate of an approved practical nursing program
- Valid Driver's License
- Reliable Transportation
- Current Automobile Insurance
- Ability to pass background screening
- Ability to maintain confidential information
