Position Summary
The Clinical Coordinator serves as a key member of the Home Health operations team, supporting clinical staff, patients, referral sources, and leadership to ensure efficient coordination of care and exceptional patient outcomes. This position is primarily office-based with flexibility for remote work as approved by leadership and operational needs.
The Clinical Coordinator acts as a liaison between patients, clinicians, physicians, hospitals, and office staff while assisting with daily clinical operations, patient care coordination, hospitalization prevention initiatives, and branch support activities.
Essential Duties and Responsibilities
Including but not limited to:
Clinical Coordination
- Track and monitor patient hospitalizations, emergency room visits, and care transitions.
- Conduct follow-up activities related to hospitalizations and changes in patient status.
- Perform high-risk patient outreach calls to identify concerns, reinforce care plans, and reduce avoidable hospitalizations.
- Review incoming laboratory results and ensure timely communication to physicians and clinical staff.
- Fax laboratory results and other clinical documentation to physicians and appropriate healthcare providers as required.
- Review, process, and enter physician orders received by the agency, ensuring accuracy and timely completion.
Patient and Provider Communication
- Triage incoming telephone calls and clinical concerns received by the office.
- Communicate with patients, caregivers, physicians, hospitals, assisted living facilities, and other healthcare providers regarding patient care needs.
- Escalate clinical concerns to agency leadership and clinicians as appropriate.
Operational Support
- Coordinate ordering of medical supplies and durable medical equipment (DME) for patients.
- Assist with intake processes, referral management, and patient admissions as needed.
- Assist with scheduling visits and resolving scheduling challenges to ensure continuity of care.
- Support branch operations during periods of increased volume or staffing shortages.
Leadership Support
- Provide branch coverage and operational support during the absence of Assistant Clinical Directors or other designated leaders.
- Assist leadership with workflow management, patient care coordination, and special projects.
- Support agency initiatives focused on quality outcomes, patient satisfaction, regulatory compliance, and hospitalization reduction.
Qualifications
- Licensed Practical Nurse (LPN) with relevant home health experience preferred.
- Previous home health experience strongly preferred.
- Knowledge of Medicare home health regulations and clinical documentation requirements preferred.
- Strong organizational, communication, and problem-solving skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Proficiency with electronic medical records (Wellsky preferred) and Google Workspace applications.
- Ability to work independently while maintaining strong collaboration with team members.
Work Environment
- Primarily office-based position.

- Occasional travel to branch locations may be required.
Success Measures
Successful Clinical Coordinators will:
- Support timely processing of physician orders and clinical documentation.
- Promote effective communication between patients, clinicians, and providers.
- Assist in reducing avoidable hospitalizations through proactive patient outreach and follow-up.
- Contribute to positive patient satisfaction outcomes and HHCAHPS performance.
- Support efficient branch operations and continuity of care.
Pay: $25.00 - $30.00 per hour
