Key Responsibilities
- Evaluate residents and develop individualized treatment plans.
- Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence.
- Assess patient progress and modify treatment plans as needed.
- Collaborate with interdisciplinary team members to ensure optimal patient outcomes.
- Educate patients, families, and caregivers on treatment goals, home exercise programs, and safety techniques.
- Maintain accurate and timely documentation in compliance with facility and regulatory standards.
- Participate in care planning and discharge planning activities.
Qualifications
- Active California Physical Therapist License.
- Graduate of an accredited Physical Therapy program.
- SNF experience preferred, but not required.
- Strong clinical, communication, and documentation skills.
- Ability to work effectively in a patient-centered, team-oriented environment.
