Summary/Objective
Works with individuals, groups and communities to improve mental health and behavior. Provides therapy to clients in and out of the school environment as well as in the home. Creates and updates diagnostic assessments, treatment plans and treatment plan reviews.
Essential Duties and Responsibilities

Deliver in-home or in-office services to clients as requested by the area mental health including the specific hours requested. Services may include going into the school.

Provide therapeutic interventions to the identified client in order to enhance and foster the client’s optimal functioning and interactions in the home and community.

To continually assess client needs, service availability and appropriateness.

Work cooperatively with treatment team members and the family in the provision of services to meet the goals developed in the Person Centered Plan.

Provide appropriate documentation for service delivery including treatment goals, progress notes and billing and submit said documentation as specified by Medicaid and CARF standards.

Maintain required billable hours. Work minimum of one evening per week, with evenings being 10 am – 6pm or 11am – 7pm, discretion upon supervisor.
- Maintain productivity of 70% of worked hours

Assist in creating and maintaining group therapy sessions

Complete and maintain all continuing education and licensure

Maintains quality results by following agency standards.

Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs.

Serves and protects the agency by adhering to professional standards, agency policies and procedures, federal, state, and local requirements, and CARF standards.

Perform other duties, as assigned, to the need of the agency deemed necessary and appropriate
(including but not limited to completion of Social Security forms, authorization or billing forms, phone calls, and participating in case consultation meetings with schools).
Qualifications, Education and Experience

Licensed Social Worker or Counselor

At minimum one (1) year of experience and education

Ability to work with people and work under pressure.

Ability to work a flexible schedule

Excellent time management, organization, attention to detail, quality focus, professionalism, productivity, thoroughness, coordination, deadline-oriented, internal communications, reporting skills.

Demonstrates knowledge of principles, methods and procedures for diagnosis treatment and rehabilitation of mental dysfunction

Works under the close supervision of an immediate clinical supervisor.
