Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to ensure accurate clinical documentation.
- Administer medications, including injections when need be and following strict protocols and safety standards.
- Provide specialized care for patients with complex conditions such as Alzheimer’s disease, dementia, or chronic pain management.
- Educate patients and their families on disease management, medication administration, and health maintenance strategies.
- Collaborate with physicians and other healthcare professionals to develop and update individualized care plans.
- Manage wound care, catheterization, tube feeding, and other skilled nursing procedures with precision and aseptic technique.
- Document all patient interactions accurately using electronic health records (EHR) systems while maintaining strict compliance with HIPAA regulations. Also documentation must be timely in order to maintain employment.
Requirements
- Valid state license as a Licensed Practical Nurse (LPN) with current certification in basic life support (BLS).
- Proven experience in home care settings or related fields such as long-term care, assisted living, or skilled nursing facilities.
- Critical care experience or hospital-based nursing experience preferred, especially in ICU or emergency room environments.
- Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth for efficient documentation.
- Knowledge of ICD-9/10 coding, medication administration protocols, and infection control practices.
- Ability to perform detailed patient assessments and vital sign monitoring accurately.
- Strong communication skills to triage patient needs effectively and educate families on complex medical procedures.
- Working knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
