Overview
Shift Requirements: M-F 8am-5pm (Some weekend availability would be great- 1 weekend per month/1 on call night per week)
License Requirements: Florida License required, will accept compact license.
Mileage Reimbursement: 0.43 per mile
County Covered: Escambia, Santa Rosa, Okaloosa, Walton
Preferred Skills: OASIS/HCHB (A MUST), wound care, PICC/IV care, lab collection, drain care, Otmony care, and experience with post-surgical care is helpful!
Responsibilities:
- Conduct comprehensive assessments of high-risk patients, develop individualized care plans with primary care providers, and monitor progress to ensure appropriate, cost-effective care.
- Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals while arranging non-medical support services such as housing and transportation to improve treatment adherence.
- Engage specialty resources as needed, promote preventive care, identify health improvement opportunities, and intervene early to reduce adverse patient outcomes.
- Maintain accurate documentation of clinical, functional, and financial outcomes while safeguarding patient privacy, confidentiality, and serving as a patient advocate.
- Qualifications: Associate's or Bachelor's degree in Nursing or related field, current unrestricted RN license, strong clinical decision-making skills, proficiency in Home Care Home Base/PointCare and OASIS documentation, serving patients in Okaloosa and Walton counties.
