Old Fort Family Medicine is seeking a full-time Physician Assistant (PA-C) or Nurse Practitioner (NP) to join our growing independent family medicine practice in Western North Carolina.

The Position
This is a full-time outpatient family medicine position caring for an established and growing patient population.
Responsibilities include:
· Preventive and wellness care
· Acute and same-day visits
· Chronic disease management
· Behavioral health management within primary care
· Medication management
· Ordering and interpreting laboratory and diagnostic testing
· Patient education and shared decision-making
· Coordination with specialists and other members of the care team
· Proactive management and follow-up of patients between traditional office visits
· Maintaining accurate and timely clinical documentation

About Our Practice
Old Fort Family Medicine is an independent primary care practice serving Old Fort and the surrounding Western North Carolina community.
Our practice includes 10 exam rooms, an on-site laboratory, and an experienced clinical and administrative team.
We provide broad-spectrum family medicine with an emphasis on accessibility, continuity, preventive care, chronic disease management, and long-term relationships with our patients.

Candidates should:
· Be a licensed or license-eligible PA-C or NP in North Carolina
· Enjoy comprehensive outpatient family medicine
· Be comfortable managing common acute and chronic medical conditions

Old Fort is located at the base of the Blue Ridge Mountains, minutes from Black Mountain and approximately 30 minutes from Asheville.
