Job Overview
We are hiring a Lead Caregiver (CNA) for a field-based support and training role focused on mentoring caregivers, reinforcing care standards, and ensuring consistency in how care is delivered.
This role is best suited for a CNA who takes pride in high-quality care, enjoys supporting other caregivers in the field, and values stability in a meaningful long-term role.
You will continue providing hands-on care when needed, but your primary focus will be supporting, coaching, and guiding caregivers in real-time field situations.
Location: South Sonoma County (Petaluma, Rohnert Park, Cotati, and surrounding areas)
Key ResponsibilitiesCaregiver Mentorship & Support
- Mentor and guide caregivers during onboarding and early employment, especially those with limited experience
- Report incidents, emergencies, and caregiver/client updates to the Care Manager
- Provide weekly check-ins with the Care Manager
- Reinforce care plans and documentation accuracy
- Help caregivers identify and report changes in client condition
- Conduct occasional observational visits to ensure quality of care
- Provide fill-in coverage in emergencies when clients cannot be left alone
Training & Development
- Provide hands-on coaching in clients’ homes as needed
- Support caregiver skill development and reinforce care standards
- Help caregivers build confidence in daily care delivery (ADLs, dementia care, transfers, etc.)
Operational Field Support
- Support caregivers with essential care tools and documentation technology, including hands-on training on how to properly use tablets for clock-in, clock-out, and care notes
- Ensure caregivers are confident and competent using the tablet system in the field
- Pick up and drop off tablets between clients and the office as needed (mileage reimbursed)
- Provide field-based troubleshooting and support for caregiver operational needs
Administrative Support (Secondary Role)
- Assist with preparing assessment packets or documentation as requested by the Care Manager
- May work occasionally from the Oakmont office in Santa Rosa
- Organize caregiver training materials and field resources
- Administrative tasks are secondary to field-based caregiver support
Communication Support
- Act as a communication bridge for non-urgent caregiver questions to the Care Manager or scheduler
- Provide clarification on procedures (no supervisory authority)
- Ensure client care plans are being followed in the field
Role Scope
This is a field-based support and training role focused on ensuring caregivers deliver consistent, safe, and high-quality care aligned with Sequoia Senior Solutions standards.
The Lead Caregiver works under the direction of the Care Manager and provides hands-on field support, including caregiver mentorship, training, and observational quality checks.
The goal of this role is to strengthen caregiver performance, improve retention, and ensure consistent quality of care across South Sonoma County, including Petaluma, Rohnert Park, and Cotati.
This role does not include supervisory, scheduling, disciplinary, or care plan development authority. It is a support-focused field role.
The primary focus is field presence supporting caregivers in real time and reinforcing company standards.
Schedule
Schedule may vary depending on client needs and coverage requirements.
You may be asked to fill in for last-minute shifts as needed. Flexibility and reliability are essential, as client care needs can change quickly.
Qualifications
- Active CNA certification (required)
- Minimum 2+ years caregiving experience
- Experience with high-acuity clients (dementia, hospice, transfers preferred)
- Strong understanding of Activities of Daily Living (ADLs)
- Reliable transportation and ability to travel between clients
- Strong, consistent work history
- Calm, professional, and dependable under pressure
- Comfortable using tablets and digital documentation systems (training provided)
