We are seeking a Nurse Practitioner who is passionate about serving vulnerable populations, values meaningful patient relationships, and thrives working independently while being part of an exceptional healthcare team.
The Nurse Practitioner is an Advanced Practice Registered Nurse (APRN) responsible for providing comprehensive primary care services to patients in their homes and community settings.
This position requires excellent clinical judgment, outstanding communication skills, and the ability to work independently while collaborating closely with physicians, medical assistants, and community partners.

- Perform comprehensive patient assessments, histories, and physical examinations.
- Diagnose acute and chronic medical conditions.
- Develop individualized treatment plans.
- Order, interpret, and communicate laboratory and diagnostic testing.
- Prescribe medications and non-pharmacologic treatment plans within scope of practice.
- Manage chronic diseases including hypertension, diabetes, COPD, CHF, dementia, and other complex medical conditions.
- Evaluate changes in patient condition and adjust treatment plans accordingly.
- Perform appropriate in-home procedures within scope of practice.
- Coordinate preventive care and wellness services.
As a Mobile Medical Group provider, you will:
- Travel daily between patient homes, assisted living facilities, memory care communities, and independent living communities.
- Provide compassionate bedside care in non-traditional healthcare settings.
- Build relationships with patients, caregivers, family members, and facility staff.
- Coordinate care with home health agencies, hospice providers, specialists, pharmacies, therapists, and other healthcare professionals.
- Educate patients and caregivers regarding diagnoses, medications, disease management, and preventative care.
- Maintain complete, accurate, and timely electronic medical records.
- Document all patient encounters according to CMS, Medicare, and organizational standards.
- Complete documentation within established timeframes.
- Ensure appropriate coding and documentation supports billing compliance.
- Review and update medication lists, allergies, diagnoses, and care plans at every visit.
- Coordinate follow-up appointments and ongoing treatment plans.
- Facilitate admissions, transfers, and discharges when appropriate.
- Collaborate with physicians and interdisciplinary healthcare teams.
- Make appropriate referrals to specialists and ancillary healthcare providers.
- Participate in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Transitional Care Management (TCM) initiatives when appropriate.
- Maintain compliance with all state and federal regulations.
- Follow evidence-based clinical guidelines.
- Protect patient confidentiality and maintain HIPAA compliance.
- Meet documentation, productivity, and quality metrics established by Mobile Medical Group.
- Participate in quality improvement initiatives and continuing education.

- Master's Degree in Nursing (MSN) from an accredited Nurse Practitioner program.
- Current Nurse Practitioner license.
- Current DEA registration (or ability to obtain).
- Current BLS Certification.
- Valid driver's license with reliable transportation.
- Minimum of five (5) years of nursing experience preferred.
- Previous primary care, family practice, geriatrics, internal medicine, hospice, home health, or skilled nursing experience strongly preferred.
- Experience caring for medically complex geriatric patients is highly desirable.
- Strong clinical assessment and diagnostic abilities.
- Excellent critical thinking and clinical judgment.
- Experience with Electronic Health Records (EHR).
- Exceptional communication and interpersonal skills.
- Ability to prioritize and work independently.
- Strong organizational and documentation skills.
- Comfortable working in a mobile healthcare environment.
