ANCHOR Care Manager
Seeking a full-time (37.5 hours/week) ANCHOR Care Manager for a hybrid role with two in-office days per week. This position helps eligible older adults with recurring behavioral health needs access and maintain services that support quality of life, independence, dignity, and
respect.
QUALIFICATIONS:
Bachelor’s Degree preferred.
Field of degree in social services or related areas from an accredited college or university preferred.
Behavioral Health experience preferred.
Strong skill set with working with individuals with complex Long-Term Services and Supports needs and challenges as it relates to effective service delivery required.
At least 1 year of experience in human services, preferably with the older adult population or transferable skills required.
COMPENSATION AND BENEFITS:
A hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year of active employment. The bonus for this position is $1,000.
