Exodus PCS is seeking a Program Supervisor to lead and grow our Substance Use Disorder (SUD) Housing and Transition Services Program. This leadership role combines staff supervision, case oversight, referral development, hiring, training, and community outreach to ensure high-quality services for individuals transitioning from treatment settings into the community.
This is a hybrid position with a combination of office-based work, remote work from home, and travel to SUD treatment centers and community partners throughout Minnesota.

Position Summary
The Program Supervisor provides leadership and oversight of the SUD program while supporting and developing a team of Case Managers. This role is responsible for ensuring case quality, maintaining referral relationships, coordinating enrollments, recruiting and training staff, and helping expand program reach throughout Minnesota.
The ideal candidate is an organized leader who enjoys mentoring others, building relationships, solving problems, and developing innovative approaches to service delivery.
Key Responsibilities
Case Management Leadership & Oversight
- Provide supervision, direction, and support for SUD Case Managers.
- Review cases for quality, compliance, and progress toward housing outcomes.
- Lead regular case consultation meetings and staff supervision meetings.
- Assist staff with complex cases, housing barriers, and discharge planning.
- Ensure documentation standards, timelines, and program requirements are met.
- Monitor caseload distribution and staffing needs.
Referral Coordination & Relationship Management
- Serve as a primary contact for treatment centers, referral partners, and community providers.
- Develop and maintain strong relationships with SUD treatment facilities across Minnesota.
- Coordinate referrals, enrollments, and service assignments.
- Respond promptly to referral inquiries and program questions.
- Identify opportunities for increased referrals and service expansion.
Marketing & Program Development
- Promote Exodus PCS services to treatment providers, counties, and community partners.
- Conduct outreach visits and presentations to referral sources.
- Develop new partnerships that support program growth.
- Help identify opportunities for new services, program enhancements, and operational improvements.
- Track referral trends and recommend growth initiatives.
Staff Hiring, Training & Development
- Participate in recruitment, interviewing, and hiring activities.
- Lead onboarding and training for new staff.
- Provide coaching, mentoring, and professional development opportunities.
- Create training resources and best practice guides.
- Foster a positive, collaborative, and accountable team culture.
Operational Leadership
- Assist with program planning, workflow management, and quality improvement initiatives.
- Monitor service outcomes and identify opportunities for greater efficiency and effectiveness.
- Collaborate with agency leadership regarding program growth and strategic goals.
- Participate in leadership meetings and organizational projects.
Qualifications
Required
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field (or equivalent combination of education and experience).
- Minimum of three years of experience in case management, social services, behavioral health, housing services, or a related field.
- Strong organizational and communication skills.
- Valid driver's license and reliable transportation.
- Ability to travel to treatment centers and community partners throughout Minnesota.
Preferred
- Previous supervisory or leadership experience.
- Experience working within SUD treatment, behavioral health, housing, transition services, or community-based programs.
- Knowledge of Minnesota social service systems and community resources.
- Experience with staff training, program development, or business development.
- Experience building referral relationships and community partnerships.
