Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately, including IV infusions and catheter care, following strict safety protocols and HIPAA regulations.
- Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures to support accurate diagnoses.
- Manage acute pain and chronic conditions through tailored nursing interventions, including ventilator management and wound care.
- Provide specialized care for patients with dementia or Alzheimer's disease, including memory care and behavioral health support.
- Support emergency response efforts by managing airway management, first aid, CPR certification maintenance, and triage procedures in urgent or trauma situations such as Level I Trauma Center or Level II Trauma Center environments.
- Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care strategies to ensure seamless patient transitions across settings.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
- Proven experience working with geriatric patients in long-term care facilities or skilled nursing facilities preferred.
- Demonstrated proficiency in EMR/EHR systems like Epic, Cerner, or Athenahealth; experience with medical documentation is essential.
- Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10).
- Experience in specialized areas such as hospice care, infectious disease care, ICU or emergency medicine is highly desirable.
- Skilled in performing physical examinations, venipuncture (phlebotomy), catheterization, tube feeding administration, and sterile techniques.
- Ability to work effectively with individuals with disabilities or developmental challenges; experience in dementia or memory care is a plus.
- Excellent communication skills for caregiver interaction and patient education; ability to handle high-pressure situations confidently.
