Job Overview
We are seeking a dedicated and compassionate Licensed Social Worker to join our workers’ compensation case management team. In this vital role, you will provide comprehensive social work services to patients across various settings, including hospitals, outpatient clinics, and community-based programs. Your expertise will help patients and their families navigate complex emotional, social, and medical challenges, ensuring they receive personalized support and care. Duties
- Conduct thorough patient assessments to evaluate social, emotional, and environmental needs.
- Develop individualized care plans that incorporate case management.
- Coordinate and place post-acute care.
- Assist with intake procedures, medical documentation, and maintaining accurate medical records in compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams to coordinate patient care across inpatient, outpatient, assisted living, and post-acute care settings.
- Assist injured workers with return to work.
Requirements
- Valid state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent certification.
- Valid driver‘s license.
- Proven experience in hospital settings or home & community care environments preferred.
- Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and EMR systems for documentation purposes.
- Experience working with diverse populations.
- Proficiency in crisis management techniques and familiarity with HIPAA regulations to ensure patient confidentiality.
- Excellent communication skills.
- Ability to handle sensitive situations with empathy while maintaining professional boundaries in high-pressure environments.
- Ability to work independently.
- Fluency in English required. Bilingual fluency in both English and Spanish a strong plus.
- Familiarity with Microsoft Office or Microsoft 365.
