Duties
- Dispense medications accurately in accordance with physician prescriptions, ensuring compliance with all safety protocols and legal regulations
- Provide comprehensive medication counseling to patients, explaining proper usage, potential side effects, and interactions to promote optimal adherence and health outcomes
- Collaborate with physicians, nurses, and other healthcare providers to develop and adjust medication therapy plans tailored to individual patient needs
- Monitor patient responses to medications, perform medication reconciliation, and adjust treatments as necessary for maximum efficacy
- Oversee sterile compounding processes, including IV infusion preparation, maintaining aseptic technique and ensuring medication integrity
- Conduct patient assessments related to medication therapy management, including evaluating physiology knowledge, anatomy understanding, and pain management needs
- Maintain detailed documentation of medication administration, patient interactions, and clinical interventions for legal compliance and quality assurance
Qualifications
- Valid pharmacist license with current registration in the relevant jurisdiction
- Extensive hospital experience, including Level I Trauma Center or Level II Trauma Center exposure preferred
- Proven expertise in hospital medicine, critical care, ICU experience, and emergency room settings
- Strong background in managed care practices and post-residency clinical pharmacy experience
- Knowledge of physiology, anatomy, medical terminology, acute pain management, and pediatrics
- Hands-on experience with IV infusion techniques and aseptic procedures in an acute care environment
- Demonstrated ability in patient service delivery focused on patient care excellence and safety standards
- Experience working within a hospital or similar healthcare setting that involves trauma centers or pediatric units such as PICU (Pediatric Intensive Care Unit)
