Woodlawn Health is seeking an OB Navigator to guide pregnant/postpartum patients through their care journey. This grant-funded position (9/1/2026–6/30/2027) connects patients with providers, coordinates appointments and resources, identifies barriers to care, and provides ongoing support throughout pregnancy/postpartum. The role works across Rochester and Argos provider offices and collaborates with clinical staff, community partners, and outreach events.
Required: RN, LPN, or MA
Preferred: Bilingual (English/Spanish)
- Registered Nurse, Licensed Practical Nurse, Certified Medical Assistant, or Medical Assistant
- Previous OB, family practice, women’s health, pediatrics, public health, or care coordination experience is preferred.
- Basic Life Support certification or obtain within the timeframe required by hospital policy
- Experience in obstetrics, maternal-child health, family practice, public health, social services, case management, community health, or patient navigation is preferred.
- Desire and ability to educate, guide, and support pregnant and postpartum patients and families in a nonjudgmental and patient-centered manner.
- Knowledge of, or willingness to learn, local community resources including WIC, Medicaid, health department services, Nurse-Family Partnership, social work/case management, transportation, food assistance, housing, mental health, substance use, domestic violence, safe sleep, lactation support, and car seat assistance.
- Valid driver’s license and reliable transportation required to travel between provider offices, the hospital campus, and community locations as assigned.
Overview of position-
Duties will be assigned based on the employee’s credentials, licensure, certification, job classification, competency, and organizational policy.
- If filled by a RN or LPN, the employee may provide nursing assessment, nursing education, clinical care coordination, and escalation of clinical concerns within applicable scope of practice and policy.
- If filled by a MA, the employee will focus on scheduling support, patient communication, education reinforcement, resource coordination, documentation, referral follow-up, and program tracking.
- The OB Navigator will promptly report urgent clinical concerns, safety concerns, abnormal patient-reported symptoms, or other high-risk needs to the appropriate provider, licensed nurse, OB department, social work/case management, or emergency services according to situation.
- Initial point of contact for newly pregnant patients seeking OB care through Woodlawn Health.
- Receive and respond to calls, messages, referrals, or requests from newly pregnant patients, provider offices, hospital departments, and community partners.
- Assist newly pregnant patients with scheduling their first OB appointment. Also, explain what to expect at the initial visit, including paperwork, insurance information, appointment location, and follow-up needs
- Provide education and reinforcement, within scope, regarding routine prenatal care, OB appointments, prenatal education options, OB unit tours, birth planning resources, lactation support, safe sleep, car seat safety, postpartum follow-up, and maternal warning signs.
- Collaborate with providers, provider office staff, OB nurses, the OB & Nursery Director, social work/case management, public health partners, and community agencies to coordinate care and support patient needs.
- Assist patients with postpartum transition needs. This includes postpartum appointment follow-up, lactation support, newborn follow-up, pediatric care connection, maternal mental health resources, contraception follow-up when applicable, safe sleep reinforcement, and ongoing community support needs
