Title: Hospice PRN Nurse
Position Summary
The PRN nurse provides skilled nursing care to hospice patients on an as needed basis. The RN works with the interdisciplinary team to ensure compassionate, high-quality end of life care while following physician orders, agency policies, and Medicare guidelines. This position will primarily support patients throughout our Southeast Houston coverage area.
Coverage Area
This position primarily serves patients throughout Southeast Houston, including but not limited to League City.
Essential Duties and Responsibilities
- Perform admissions, routine, PRN, and follow up nursing visits.
- Complete comprehensive patient assessments and develop or update the plan of care.
- Provide skilled nursing care and symptom management.
- Administer medications and treatments as ordered.
- Educate patients and families regarding disease progression, medications, and end of life care.
- Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, and other team members.
- Participate in interdisciplinary group (IDG) meetings as requested.
- Document all visits accurately and timely in the electronic medical record.
- Respond to urgent patient needs when scheduled.
- Ensure compliance with Medicare, state, and agency regulations.
- Report changes in patient condition to the clinical supervisor and physician.
- Maintain patient confidentiality in accordance with HIPAA.
Qualifications
- Current Registered Nurse (RN) license in the state of Texas.
- Graduate of an accredited nursing program.
- Minimum of one year of nursing experience preferred.
- Hospice or home health experience preferred.
- Current BLS certification.
- Valid driver’s license, reliable transportation, and car insurance.
- Ability to travel throughout the assigned Southeast Houston service area with reliable transportation.

Physical Requirements
- Ability to lift up to 50 pounds with assistance when necessary.
- Frequent standing, walking, bending, and driving.
- Ability to provide direct patient care in home settings.
Schedule
- PRN (As Needed)
- Flexible schedule based on patient census and staffing needs.
- May include weekdays, weekends, holidays, and on-call coverage as needed.
