Job Title: RN Home Visiting Nurse

Position Overview:
This role combines both in-office responsibilities and fieldwork, providing essential clinical support to patients in their service locations. The Clinical Nurse is responsible for conducting patient visits for various care milestones, supporting ongoing clinical needs, and ensuring the highest standard of care is delivered in compliance with clinical guidelines.
Key Responsibilities:
- Conduct in-person patient visits for:
- Admissions – New patient onboarding and initial evaluations
- Recertifications – Comprehensive assessments every 60 days
- Resumptions of Care – Following hospital discharge or care interruption
- Supervision Visits – Ongoing assessments every 14 days to ensure quality of care and compliance
- Other visits-Directed by the administrator or Clinical Manager
- Respond to daily clinical inquiries from field staff, physicians, and patients
- Document all clinical findings and patient interactions accurately and in a timely manner
- Collaborate with interdisciplinary teams to update care plans and coordinate services
- Ensure compliance with federal, state, and organizational standards for patient care
- Attend internal meetings and training sessions as required
Qualifications:
- Current Registered Nurse (RN) license in the state of Pennsylvania
- Minimum 1 year of clinical nursing experience, preferably in home health or community-based care
- Strong clinical assessment, critical thinking, and communication skills
- Ability to work independently and manage a flexible field-based schedule
- Must have a reliable vehicle, valid driver’s license, and willingness to drive to patient service locations
Work Environment:
- Office-based administrative and clinical tasks
- Field visits to patient homes or service locations (travel required)
- Use of electronic medical records (EMR) system for documenting assessments
