Responsibilities
- Core Responsibilities
- Provide respectful, person-centered support to adults with intellectual disabilities during day habilitation activities.
Health, Safety & Behavior Support
- Monitor participant health and safety; report changes, incidents, or concerns promptly to supervisors and document as required.
- Use approved behavior support plans and de-escalation techniques; document incidents and follow reporting procedures.
- Administer or assist with medication only if trained and authorized; follow all medication policies and protocols.
- Maintain clean, safe program spaces and follow infection control and emergency procedures.
Documentation & Communication
- Accurately complete daily progress notes, attendance logs, incident reports, and other required documentation in a timely manner.
- Communicate respectfully and regularly with participants’ families, caregivers, and interdisciplinary team members about progress and concerns.
- Participate in team meetings, case conferences, and ISP reviews, contributing observations and recommendations.
Professionalism & Training
- Attend required trainings (CPR/First Aid, person-centered planning, behavior supports, mandated reporting) and maintain certification currency.
- Preserve participant confidentiality and uphold agency policies, ethics, and regulatory requirements.
- Demonstrate cultural competence, respect for diversity, and commitment to participant choice and dignity.
Community & Transport (if applicable)
- Support community outings and public transportation access, ensuring participant safety and skill development.
- Assist with vocational exploration or supported employment activities as directed.
Other
- Perform light housekeeping and meal/snack preparation consistent with health and safety standards.
- Flexibility to work varied schedules as needed and assist with transitions between activities.
- Assist participants with activities of daily living as needed (personal care, toileting, grooming) while promoting independence.
- Implement individualized service plans (ISPs) and habilitation goals under supervision, using positive support strategies.
- Facilitate skill-building activities (social skills, vocational tasks, life skills, recreation) tailored to each participant’s goals and abilities.
- Lead and adapt group and 1:1 programming, ensuring engagement, inclusion, and meaningful community participation.
Experience
- Demonstrated ability to work with individuals with developmental disabilities
- Knowledge of patient observation techniques, patient monitoring, personal care best practices.
- Experience with medication administration, first aid certification, CPR training, and manual handling equipment such as Hoyer lifts is advantageous. Will Train
