Key Responsibilities
- Case Management Support: Supervise and maintain accurate, up-to-date case management notes addressing personal issues and workplace situations affecting staff, including those related to patient care.
- Staff Support: Serve as a primary point of contact for staff, addressing concerns with discretion, empathy, and in accordance with company policy.
- Communication Liaison: Act as a communication bridge between care teams, supervisors, and clients, ensuring information flows accurately and confidentially.
- Issue Resolution: Identify and address problems as they arise, escalating matters appropriately to supervisors or management.
- Compliance and Documentation: Ensure all records and documentation meet organizational standards for accuracy, thoroughness, and confidentiality.
- Scheduling Assistance: Collaborate on scheduling to ensure appropriate coverage and efficient allocation of staff resources.
- Continuous Improvement: Proactively suggest process improvements to enhance care quality and staff satisfaction.
Required Skills, Qualification and Experience
- Strong organizational and time-management skills.
- One year experience as a Coordinator
- GED or High School Diploma (Bachelor’s degree preferred)
- Excellent written and verbal communication abilities.
- Proficiency in maintaining accurate records and handling confidential information.
- Problem-solving attitude with an empathetic, team-oriented approach.
- Experience in home care, healthcare administration, or a related field is an advantage.
- Comfort using scheduling and documentation software (training provided if needed).
- The role requires proficiency in using a personal computer, calculator, multi-line telephone system, cell phones, and other standard office equipment.
