About the Role
Silver Oak Nursing & Rehabilitation Center is seeking a compassionate and motivated Occupational Therapist (OT) to join our in-house subacute rehabilitation team. In this role, you’ll evaluate and treat residents to improve their ability to perform activities of daily living and achieve the highest level of independence possible in a skilled nursing and subacute rehab setting.
You’ll collaborate closely with an interdisciplinary team to support residents’ functional recovery, safety, and quality of life.
Key Responsibilities
- Evaluate residents’ functional abilities, including ADLs, mobility, strength, coordination, cognition, and safety awareness
- Develop and implement individualized treatment plans per physician orders
- Provide skilled occupational therapy services to short-term rehab and long-term care residents
- Complete evaluations, progress notes, and discharge summaries accurately and timely
- Educate residents, families, and caregivers on treatment goals, progress, and home safety strategies
- Collaborate with nursing, PT, SLP, physicians, and other interdisciplinary team members
- Participate in care plan meetings and interdisciplinary team conferences
- Maintain compliance with state, federal, and facility regulations
Qualifications
- Master’s degree in Occupational Therapy from an accredited program
- Current Iowa OT license or eligibility
- NBCOT certification preferred
- Skilled nursing or subacute rehab experience preferred (new graduates welcome)
- Strong communication, documentation, and organizational skills
- Ability to work independently and collaboratively
