Education –
Experience –

Previous experience in adult long term care preferred.

Two or more years of experience working in a residential facility.

Experience in individual/family counseling, group facilitation, admission and discharge planning and care coordination.
Skills –

Individual, family and group counseling

Ability to provide case coordination, admission and discharge planning with an awareness of available community services.

Ability to work as part of an interdisciplinary care team

Ability to set priorities and work independently

Ability to accept and utilize constructive feedback to enhance professional skills

Knowledge of the MDS, depression and anxiety tools desirable
Physical and Mental Requirements:
1. Ability to read and write at the level of Grade 12 or above.
2. Must be able to hear alarms and intercom pages.
3. Must be able to communicate orally and in writing.
