Responsibilities
Nursing Assessment:
- Provides comprehensive, accurate nursing assessments to determine mental health, substance abuse, depression, and medical needs in both an outpatient clinic setting and via telemedicine.
- Appropriately performs steps in obtaining vital signs and measurements and records each accurately.
- Assists providers with telemedicine visits by obtaining and updating the patient’s medication list, past medical and surgical histories, family history, and hospitalizations.
- Performs and interprets laboratory tests such as Rapid HIV and HCV tests, urine pregnancy tests, urine drug screenings, and glucometer readings.
- Observes patients; records significant conditions and/or reactions; and notifies supervisor or physician of changes in patients’ condition, reaction to drugs, treatments, or significant incidents.
- Documents services, telephone encounters, patient contacts, and other relevant information accurately, in the approved formats and locations in the electronic health record.
- Responds to patient, staff member, or visitor medical emergencies and acts according to SCLHSA policies. Performs CPR, applies the Automated External Defibrillator, and provides first aid as needed in the event of a medical emergency.
Coordination of Care:
- Facilitates processes to assist the medical staff/prescribers in managing their schedules and coordinates services, including referrals as needed.
- Collaborates with SCLHSA Primary Care Team, as well as outside primary care providers for integrated care.
- Coordinates with SCLHSA Pharmacy Department, private pharmacies, and third-party insurers to relay prescription information, obtain authorizations, and monitor medication utilization.
- Coordinates services between agencies such as emergency medical transport, hospitals, nursing homes, and law enforcement as they pertain to client healthcare.
- Ensures documentation is concurrent with each service and/or receipt of updated information.
- Attends staffing meetings focused on the welfare of patients.
Medication Management:
- Provides appropriate and accurate medication management services including medication administration, maintenance and security of on-site medications, and medication returns to the SCLHSA Pharmacy.
- Adheres to the Louisiana State Boards of Nursing and Pharmacy regulations regarding storage, security, distribution, and administration of medications.
- Maintains medication supplies by monitoring stock levels and expiration dates, facilitating orders, and connecting clients with patient assistance programs.
Education:
- Participates in client and family health teaching and discharge planning.
- Provides medication education and support to clients and families receiving medication.
Infection Control:
- Consistently and appropriately uses universal precautions during service delivery.
- Consistently and appropriately uses accepted infection control measures including correct use of protective gear, disposal of bio-hazardous waste and sharps, and hand washing.
Other duties as assigned. Attends staff meetings. May assist with other agency functions including, but not limited to, data entry, reports, program development, staff training, staff orientation, chart audits, etc.
Minimum Qualifications
- Possession of a current Louisiana Registered Nurse license or a temporary permit or multi-state license issued by a Nursing Licensure Compact (NLC/eNLC) state to practice as a registered nurse plus one year of experience as a licensed registered nurse
Pay: $30.00 - $35.00 per hour
