The Clinical Liaison serves as the primary connection between referral sources and the Long-Term Acute Care Hospital (LTACH). This role is responsible for evaluating potential patient referrals, ensuring appropriate admissions, and building strong relationships with hospitals, physicians, and case management teams. The Clinical Liaison plays a critical role in promoting the hospital’s services while ensuring patients are transitioned to the appropriate level of care.
Key Responsibilities
- Develop and maintain strong relationships with referral sources, including acute care hospitals, case managers, physicians, and discharge planners
- Conduct on-site and/or remote clinical assessments of referred patients to determine appropriateness for LTACH admission
- Collaborate with interdisciplinary teams to review patient cases and coordinate admissions
- Educate referral sources on LTACH services, admission criteria, and patient outcomes
- Serve as a resource for discharge planning and post-acute care placement
- Manage and track referrals, ensuring timely follow-up and communication
- Participate in marketing and business development activities, including facility visits and presentations
- Maintain accurate documentation of referral activity and outcomes
- Ensure compliance with all regulatory and organizational policies
Pay: $66,560.00 - $89,000.00 per year
