We are seeking a compassionate and skilled LPN Licensed Practical Nurse to join our team at Fountains Rehabilitation at Mill Cove, a Senior Living Center nestled in the heart of Jacksonville, Florida. As a valued member of our care team, you will play a vital role in delivering exceptional healthcare services to our residents.
About the LPN Licensed Practical Nurse Role:
Provide high-quality patient care: As an LPN, you will be responsible for assessing, planning, implementing, and evaluating patient care plans in collaboration with our interdisciplinary team.
Collaborate with healthcare professionals: Work closely with our physicians, therapists, and other healthcare professionals to ensure comprehensive care for our residents.
Communicate effectively: Develop strong relationships with residents, families, and caregivers by communicating effectively and empathetically.
Maintain accurate records: Accurately document patient records, including medical histories, treatment plans, and progress reports.
LPN Licensed Practical Nurse Responsibilities:
Administer medications and treatments as prescribed by physicians
Monitor and report changes in patient condition to physicians and other healthcare professionals
Perform various diagnostic tests, such as vital signs and blood pressure checks
Assist with wound care, dressing changes, and other medical procedures
Provide emotional support and education to residents and families
