Shift: Monday – Friday, 8:30A – 3:30P
Salary: $20.01/hr

Qualifications:
- Clean and valid NYS Driver’s License required
- High School Diploma/GED preferred, but not required
- AMAP Certification preferred
- HHA experience preferred
Responsibilities:
- Drive program participants to and from community outings and other places as needed.
- Assist the Community Coach in facilitating dialogue with program participants at site consistent with the integration of activities and learning modules.
- Ability to assume responsibility at site to include, but not limited to, supervision of program participants and the collection of data for documentation in absence of Community Coach
- Assist with implementation and recording of data consistent with identified valued outcomes and informal outcomes.
- Report situations/issues involving consumers to proper authorities.
- Assist program participants with all ADL needs including, but not limited to, feeding, transferring, lifting, toileting, dressing (coats), etc. as per person’s protocols.
- Ensure the needs of program participants are met including but not limited to their safety and well-being.
- Ensure program participants appointments and schedules are adhered to.

Department Details:
Life Options (LO), an innovative consumer-driven program, offers adults the opportunity to find achievement and satisfaction through participation in learning modules where consumers choose activities of interest. Community inclusion activities augment the learning modules and prevocational (paid) work activities.
Key Responsibilities include, but not limited to:
- Provide transportation assistance to consumers according to their schedules within the learning module classroom days.
- Assist consumer with all ADL needs including, but not limited to, feeding, transferring, lifting, toileting, dressing (coats), etc. as per consumer’s protocols.
- Ensure needs of consumers are met including but not limited to their safety and well-being.
- Ensure consumer appointments and schedules are adhered too.
- Ensure compliance with all OPWDD/OPTS regulations and Life Options’ Policies & Procedures, to include but not limited to training on behavioral plans/strategies & protocols.
- Contact families as needed.
- Responsible for doing bus duty.
- Report situations/issues involving consumers to proper authorities.
- Incorporate Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.
Physical Demands:
- Must be able to bend, lift and push more than 100 pounds.
- Must be able to stand and walk for extended periods of time.
- Provide hand-over-hand assistance in classroom when necessary.
