Lillia is hiring a part-time, contract U.S.-licensed Registered Nurse (RN), New Jersey license required, to support remote chronic care programs for primary care practices in the United States, beginning with a diabetes and continuous glucose monitoring (CGM) pilot in New Jersey.
This is a primarily remote, process-driven care coordination role focused on patient engagement, adherence, documentation, and escalation support for chronic disease populations, especially Medicare patients. Depending on the needs of partner practices, the RN may occasionally provide on-site support for patient onboarding, workflow coordination, staff collaboration, and program implementation within New Jersey.
The role is ideal for nurses who are entrepreneurial and enjoy structured patient interaction, technology-enabled care, and operational excellence rather than high-acuity bedside work. The role starts as part-time, leading towards full-time.
Key responsibilities
● Conduct structured patient check-ins and coaching touchpoints across the program journey (cadence varies by protocol)
● Support chronic disease education, adherence, and care coordination
● Monitor patient-reported data and RPM alerts
● Document patient interactions accurately for quality and evidence capture for billing compliance
● Follow structured scripts, workflows, and escalation protocols
● Escalate clinical concerns appropriately to supervising providers or NPs
● Maintain high-quality patient engagement and empathy, especially with elderly populations
● Collaborate with Lillia’s operations and AI-assisted workflow systems
● Help optimize operational workflows and patient experience over time
● Serve as the single day-to-day operational point of contact for the partner clinic.
● Become an early expert user of the Lillia platform and help stand up and refine our first US workflows.
● Support diabetes care specifically, including CGM data review and glucose-pattern awareness.
Ideal candidate profile
● Active New Jersey RN license required (Nurse Licensure Compact / multi-state a plus)
● Experience in primary care, chronic care, care coordination, telehealth, or population health
● Comfortable with remote work and digital communication tools
● Tech-savvy and open to AI-assisted workflows
● Strong communication and patient engagement skills
● Reliable, organized, and highly process-oriented
● Comfortable with repetitive monthly patient interactions
● Strong documentation discipline and compliance mindset
● Trainable and adaptable to evolving workflows
● Entrepreneurial mindset with comfort operating in a fast-moving startup environment
● Proactive, resourceful, and comfortable helping shape new workflows and operational processes
Preferred experience
● CCM and/or RPM experience
● Medicare patient population exposure
● Experience using EHRs, care coordination platforms, or digital health tools
● Experience in remote care environments
● Diabetes care, endocrinology, or CGM/glucose-data exposure
