Definition of Position:
Provides coordination and support for Community Health & Outreach programs and departments.
Responsibilities:
Essential Job Functions:

Provides customer service for all Community Health & Outreach departments (phone calls, patient/community walk-ins, and appointments for ConneXions, Medicare counseling, community classes, and more.

Assigns all new Community Health and ConneXions referrals to the appropriate department and/or Community Health Worker.

Maintains a current, up-to-date list of all community resources and services.

Schedules, coordinates, and completes data entry and collection for all CPR/BLS classes.

Data collection and inventory management for all community benefit services.

Manages all department inventory and warehouse items.
-
Provides direct support for CareVan scheduling/appointment management.

Assists with scheduling and coordinating all community classes and events.

Gathers and organizes all necessary materials for 40+ events/year.

Coordinates staffing for each community event.

Provides support for Employee Wellness program, software, and data entry.

Assists with community health messaging/advertising in the form of bulletin boards, brochure management, event/class/health messaging social media postings, and more.

Assists with data entry, document creation, filing, tracking, reporting.

Maintains up-to-date records for all CHO programs, events, classes, and grants.

Assists with other departmental projects, as needed.

Oversee the day-to-day operation of Community Health & Outreach programs and departments by aiding as needed.

Communicate effectively and therapeutically with people from every socioeconomic background.

Participates in required education for DNV programs as applicable to position. (reference program education curriculum).

Qualifications:
Qualifications:
Education
Required: High school diploma or equivalent.
Preferred: Supplemental courses in office work preferred.
Licenses/ certifications/ registrations

Experience
Required: Customer service experience.
Preferred: Prefer 3 years’ previous experience working in an administrative/office setting. Previous event planning/management experience.

Excellent skills in English, punctuation, spelling, grammar, proofreading, attention to detail

Proficient in computer skills, typing, Microsoft programs (Word, Excel, Publisher), telephone systems, and copy/fax machines.

Bilingual English/Spanish preferred.
