The PATH (Patient Access to Health) Care Coordinator serves as the primary point of contact and front door to the Care Management department, providing high-touch customer service, patient navigation, referral coordination, and operational support to patients, caregivers, providers, health plans, and office staff.
This role assists patients in understanding health plan benefits, accessing healthcare services, scheduling appointments, obtaining referrals, and connecting with internal and community-based resources. The PATH Care Coordinator serves as a communication liaison between patients and care teams, helping reduce barriers to care and improve the overall patient experience.
