Overview
We are seeking an immediate hire for a compassionate, patient and reliable Home Health Aide with experience caring for individuals with dementia. The ideal candidate is respectful, kind, and knowledgeable about dementia related behaviors, and is comfortable providing both personal care and light household support in a home setting. In this vital role, you will provide personalized support to individuals in their homes, promoting independence and well-being through attentive caregiving. Your expertise will help clients manage daily activities, maintain health, and enjoy quality of life in a safe and nurturing environment. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring specialized memory or hospice care.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts or wheelchairs
- Monitor and document patient observations, vital signs, and medical conditions accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following prescribed protocols while maintaining compliance with HIPAA regulations for patient privacy
- Provide companionship and emotional support to clients with dementia or Alzheimer's disease, ensuring a safe environment for memory care needs Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a clean living space conducive to health and safety
- Assist with medical procedures like catheter care or hospice-related activities under the supervision of healthcare professionals
- Maintain detailed medical documentation and records to track patient progress and communicate effectively with healthcare teams
- Ability to travel between client homes as needed
- Must have reliable transportation
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated ability to work with people with developmental disabilities or those requiring specialized dementia or Alzheimer's care
- Knowledge of medical terminology and familiarity with medical documentation practices
- Certification in CPR is required; additional certifications such as medication administration or hospice care are advantageous
- Experience operating EHR/EMR systems and managing patient records securely in compliance with HIPAA standards
- Proven ability to handle patient care tasks independently while collaborating effectively within a healthcare team
- Long-term care experience and familiarity with working with diverse populations enhance your ability to deliver exceptional home health services
