Golden Agers LLC DBA Nurse Next Door Northern Virginia is seeking an experienced Registered Nurse to join our growing home health team. We are looking for a compassionate, clinically skilled RN who is experienced in providing care to patients requiring dialysis and other complex nursing services in the home.
Responsibilities:
- Conduct comprehensive Start of Care (SOC), Resumption of Care (ROC), Recertification, Supervisory, and Routine Nursing Visits.
- Perform comprehensive nursing assessments and develop individualized care plans.
- Coordinate care with physicians, therapists, caregivers, and family members.
- Provide skilled nursing care in accordance with physician orders and agency policies.
- Perform IV therapy, including IV insertion, medication administration, hydration, antibiotic infusions, and maintenance of vascular access devices.
- Care for patients receiving Hemodialysis (HD), Peritoneal Dialysis (PD), Home Hemodialysis (HHD), Continuous Cycling Peritoneal Dialysis (CCPD), and Continuous Ambulatory Peritoneal Dialysis (CAPD).
- Assess dialysis access sites, including AV fistulas, AV grafts, central venous catheters, and PD catheters.
- Educate patients and caregivers on dialysis care, infection prevention, fluid management, and recognizing complications.
- Perform PICC line care and central line dressing changes.
- Insert, manage, and change Foley catheters.
- Perform NG tube and PEG tube care.
- Provide wound assessment, wound dressing changes, and infection prevention.
- Monitor vital signs, identify changes in patient condition, and promptly communicate concerns to the physician and clinical supervisor.
- Complete accurate and timely documentation using electronic medical records.
- Maintain compliance with Medicare, CHAP, and agency standards.
- Deliver exceptional care aligned with Nurse Next Door's Happier Aging® philosophy.
Qualifications Required:
- Current Registered Nurse (RN) license in Virginia.
- Minimum 3 years of nursing experience.
- Minimum 1–2 years of dialysis, nephrology, or critical care experience preferred.
- Hands-on experience caring for patients receiving:
- Hemodialysis (HD)
- Peritoneal Dialysis (PD)
- Home Hemodialysis (HHD)
- CAPD
- CCPD
- Experience with:
- IV insertion and IV infusion therapy
- PICC lines and central venous catheters
- Foley catheter insertion and management
- NG tube and PEG tube care
- Wound care and dressing changes
- Strong assessment and clinical decision-making skills.
- Excellent communication and patient education skills.
- Ability to work independently in patients' homes.
Preferred:
- Previous experience in a dialysis center, nephrology practice, or home dialysis program.
- Minimum 1–2 years of home health
- OASIS documentation experience.
- Home Health experience.
- Infusion nursing experience.
- Wound care certification.
- Current BLS certification.
Other Requirements
- Current TB screening.
- Criminal background check.
- Valid driver's license and reliable transportation.
- Ability to legally work in the United States.
- Fluent in spoken and written English (additional languages are an asset).
- Willingness to travel throughout Northern Virginia.
