Job Summary
We are seeking a compassionate and dedicated Personal Care/Home Maker to provide essential support and assistance to individuals in their homes or community settings. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Prepare nutritious meals tailored to individual dietary needs and preferences
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates to ensure health stability
- Administer medications accurately following care plans and HIPAA confidentiality standards
- Support clients with mobility using equipment such as Hoyer lifts or transfer belts; perform heavy lifting when necessary
- Maintain a clean and safe environment through regular cleaning and organization of living spaces
- Observe and report changes in client condition or behavior, including signs of dementia or Alzheimer's disease
- Assist with light housekeeping tasks to promote a comfortable living environment
- Follow established care plans and document all services provided in accordance with healthcare regulations
- Provide companionship and emotional support to foster social engagement and reduce feelings of isolation
- Collaborate with healthcare professionals, family members, and support teams to ensure comprehensive care delivery
- Support clients during medical appointments or community outings as needed
Skills
- Proven experience in home care, assisted living, or long-term care environments
- Knowledge of patient observation techniques and vital signs monitoring
- Familiarity with Alzheimer’s care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or autism spectrum disorder
- Ability to perform CPR and First Aid confidently; certification preferred
- Experience with medication administration and understanding of HIPAA regulations for patient privacy
- Strong cleaning skills to maintain hygienic living spaces
- Ability to assist with ADLs efficiently while respecting client dignity and independence
- Excellent communication skills for effective collaboration with clients’ families and healthcare teams
- Physical stamina for heavy lifting, assisting with mobility aids like Hoyer lifts, and supporting active clients
- Compassionate attitude combined with patience, empathy, and active listening skills
- Knowledge of home health practices including social work principles is a plus
