We are seeking compassionate and reliable Home Health Aides to join our growing Private Duty and PASSPORT team. In this role, you will provide one-on-one care to patients in their homes, assisting with daily living activities and promoting independence in a safe and supportive environment. Responsibilities:
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting
- Provide support with mobility, transfers, and ambulation
- Meal preparation in accordance with patient needs
- Provide medication reminders (no administration)
- Perform light housekeeping duties
- Monitor and report changes in patient condition to the clinical team
- Provide companionship and emotional support to patients
- Follow individualized care plans and agency policies
- Tasks performed by a Home Health Aide must be assigned and directed under the supervision of a registered nurse.
- Documents care services rendered on daily progress notes and submits them to the Agency weekly.
- Reports any change in the client’s condition or in his or her home situation to the staff nurse or Director of Nursing
Qualifications:
- Previous home care or healthcare experience preferred
- Reliable transportation and valid driver’s license/insurance
- Ability to pass background check
- Strong communication and interpersonal skills
- Compassionate, dependable, and patient-focused
- HHA or STNA certification is preferred; training is available.
Organizational Relationship:
· Reports directly to the staff nurse and indirectly to the Director of Nursing.
Functional Abilities:
· Must be able to communicate verbally and in writing in a manner understood by most persons.
· Must be able to lift and transfer clients.
· Must be able to travel to client’s residences.
