Job Summary
We are seeking a dedicated Case Manager / Client Care Coordinator to support clients through a blend of care coordination and hands‑on assistance. This role is ideal for someone who is organized, compassionate, and comfortable working both in the field and in a client‑facing support role. You will collaborate with caregivers, families, and healthcare providers to ensure clients receive the highest quality of care.
Responsibilities
Case Management
· Serve as the main liaison for clients and families, ensuring satisfaction with care services.
· Carry out 7, 30, 60, 90 days visits of clients.
· Always be available to introduce clients to their new caregivers (Meet and Greet).
· Conduct initial intake calls and assist with onboarding new clients.
· Maintain ongoing communication with clients to monitor care needs and adjust as needed.
· Address and resolve client concerns promptly and professionally.
· Conduct initial and ongoing assessments to determine client needs and goals
· Develop individualized care plans and update them as conditions change
· Maintain regular communication with clients, families, and healthcare providers
· Monitor client progress and ensure care plans are followed
· Prepare and maintain accurate documentation, notes, and care records
· Support caregivers with guidance, problem‑solving, and client‑specific instructions
Client Care Coordinator Duties
· Assist with administrative duties and responsibilities.
· Assist the Nurse in training caregivers.
· Assist with seeing or visiting clients.
· Making copies and adding client’s files and helping prepare for orientations of caregivers.
· Accurately document care provided and client observations.
· Follow care plans and agency policies at all times.
· Communicate effectively with clients, families, and the care team.
Health & Wellness
· Measure and record vital signs (temperature, pulse, respiration, blood pressure, weight).
· Monitor and report changes in client condition to supervising nurse.
Companionship & Emotional Support
· Provide companionship and engage clients in meaningful activities.
· Offer emotional support to clients and families.
· Encourage independence while maintaining a safe environment.
· Support medication reminders and ensure medication lists are updated and signed
· Build trusting, supportive relationships with clients and families
Qualifications
- Experience in home care, caregiving, case management, or related field
- Strong communication and organizational skills
- Ability to work independently and manage multiple clients
- Compassionate, patient‑centered approach
- Reliable transportation, valid driver’s license, and ability to travel to client homes
- CNA, HCA, or medical background preferred

Pay: $20.00 - $23.50 per hour
